Community Support Worker Position Overview

We are seeking a compassionate and dedicated Community Support Worker to join our team. In this role, you will provide direct support, advocacy, and skill-building services to clients, helping them navigate challenges and work toward their personal goals. The ideal candidate will be empathetic, patient, and committed to making a positive impact in the lives of vulnerable individuals.

Key Responsibilities

Client Support & Case Management

Assess clients' needs, challenges, and goals through interviews and observations
Develop and implement individualized support plans aligned with clients' objectives
Monitor clients' progress and adjust interventions as needed
Maintain accurate and confidential records of all client interactions and support provided

Mental Health & Emotional Support

Provide emotional support to clients dealing with anxiety, depression, or other mental health issues
Teach coping strategies and stress management techniques
Encourage and facilitate participation in therapy, counseling, or support groups
Create a safe, non-judgmental environment for clients to express concerns

Life Skills Development

Teach essential skills for independent living, such as budgeting, meal preparation, and personal hygiene
Assist clients with job readiness, including resume writing, interview preparation, and job searching
Promote self-confidence and self-sufficiency through skill-building exercises
Support clients in developing routines and habits that contribute to stability

Crisis Intervention & Conflict Resolution

Respond appropriately to emergencies and provide immediate support
Assist clients in de-escalating crises using conflict-resolution techniques
Connect clients to emergency resources (e.g., shelters, crisis hotlines, medical care)
Implement safety planning strategies with at-risk clients

Advocacy & Resource Connection

Help clients access housing, healthcare, financial aid, and other community resources
Advocate for clients' rights and needs within social service systems
Provide referrals to relevant programs and agencies
Navigate complex systems on behalf of clients when necessary

Social & Community Integration

Encourage participation in community activities to reduce isolation
Foster the development of positive social relationships and support networks
Assist clients in developing communication and interpersonal skills
Facilitate group activities that promote social engagement

Medication & Health Management Support

Educate clients about the importance of medication adherence
Assist in scheduling and attending medical or mental health appointments
Promote healthy habits and overall well-being
Coordinate with healthcare providers as needed

Family & Caregiver Support

Work with families to strengthen relationships and improve communication
Provide education on mental health or disability-related topics
Offer guidance on how caregivers can support their loved ones
Facilitate family meetings when appropriate

Documentation & Reporting

Maintain detailed and confidential records of client progress
Prepare reports for supervisors, social workers, or case managers
Ensure compliance with organizational and governmental regulations
Document incidents and interventions according to protocols

Collaboration with Other Professionals

Work alongside social workers, therapists, doctors, and community organizations
Participate in team meetings to discuss client progress and care plans
Coordinate services to ensure comprehensive support
Contribute to the development of best practices within the team

Qualifications Required:

Knowledge of mental health, substance abuse, and social services
Strong communication and interpersonal skills
Problem-solving and crisis management abilities
Empathy, patience, and cultural sensitivity
Ability to work both independently and collaboratively
Valid driver's license and reliable transportation
CPR and First Aid certification

Preferred

Bachelor’s degree in social work, Psychology, Human Services, or related field (orequivalent experience)
Previous experience in community support or mental health services
Knowledge of local community resources and services
Experience working with diverse populations
Crisis intervention training
