Total Family Care Coalition (TFCC) is a community-based nonprofit organization in Washington, DC dedicated to supporting youth and families impacted by behavioral health challenges, trauma, and life transitions. TFCC empowers families by providing wraparound services, advocacy, mentoring, and recovery-focused programs that promote healing, growth, and resilience. Our team works collaboratively with schools, social service agencies, and community partners to ensure that every youth and family has the tools and support needed to thrive.

Position Summary

The Case Manager provides direct support services to youth and families enrolled in TFCC programs. This position plays a vital role in mentoring, case management, and advocacy, ensuring families have access to resources that foster long-term stability and recovery. The ideal candidate will be compassionate, highly organized, and dedicated to empowering individuals and families to reach their full potential.

Key ResponsibilitiesDirect Client Support

Provide one-on-one mentoring, recovery coaching, and life skills support to youth and families.
Conduct regular home visits, community meetings, and follow-ups to monitor progress and provide guidance.
Support clients in developing and implementing Individual Service Plans (ISPs) tailored to their goals and needs.
Facilitate support groups, workshops, and family engagement sessions.

Case Management & Advocacy

Link clients to community resources such as mental health services, educational programs, housing support, and employment opportunities.
Advocate for families in schools, courts, and social service systems to ensure their needs and voices are represented.
Maintain accurate documentation, including progress notes, incident reports, and outcome tracking, in compliance with program requirements.

Collaboration & Community Engagement

Work closely with clinical staff, social workers, and community partners to provide coordinated care.
Participate in multidisciplinary team meetings, trainings, and community outreach events.
Support TFCC’s mission of building stronger families and healthier communities through advocacy and engagement.

Qualifications

Required:

High school diploma or GED with lived experience in behavioral health recovery, or equivalent experience.
Strong communication, problem-solving, and relationship-building skills.
Ability to work with youth and families from diverse backgrounds with sensitivity and cultural competence.
Basic computer literacy and ability to complete timely documentation.

Preferred:

Associate’s or Bachelor’s degree in Social Work, Human Services, Psychology, or related field.
Certified Peer Specialist, Recovery Coach, or similar credential.
Previous experience working with at-risk youth, families, or community-based programs.
Bilingual (English/Spanish) a plus.

Working Conditions

Based in Washington, DC with travel to homes, schools, and community settings.
Flexible schedule may include evenings and occasional weekends.
Hybrid office/community-based work environment.
