Project Summary
Community Public Health Teams (CPHT) is a model of public health service delivery being piloted throughout Los Angeles County. As the lead organization Esperanza Community Housing is partnering with St. John’s Health Care and the LA County Department of Public health (DPH) to form a coordinated, place-based, and community-driven team that will provide research, engagement, and services to 8,000-13,000 households in designated census tracts that include the communities of Florence, Vermont-Slauson, and South Park . Using a shared leadership model, CPHT works with communities to identify local health priorities, reduce gaps in health outcomes, and improve the conditions essential for overall health and well-being.
Position: Community Health Worker
Reports To: Program Manager
Service Area: Los Angeles
Scope of Work
The Community Health Worker will engage community members in a culturally competent manner to ensure they are connected to services that meet their health needs.
Essential Duties
Community health workers will:
- Visit households in the designated area and use program provided technology to conduct a set number household needs assessments
- Conduct follow-up home visits as needed and/or as required by the program/project
- Participate in community outreach activities including canvassing and attending resource fairs and other community events
- Ensure residents are connected to health care services
- Provide systems navigation, advocacy, and education
- Provide linkages/referrals to information and support resources as requested by community members
- Identify potential sites and groups to conduct outreach and education activities.
- Prepare outreach materials as needed
- Maintain accurate and up to date documentation of client services and activities
- Maintain accurate and up to date records of outreach conducted
- Maintain constant and consistent communication with Program Manager
- Participate in required trainings including HIPAA and IRB
- Submit required quantitative and qualitative data in a timely manner
- Attend organizational, project related, and supervision meetings
- Provide support as needed with grant proposals, fundraising, grant reporting, and program evaluation activities
- Some weekends and evenings required
- Other duties as assigned
Qualifications
- Previous experience working with the target population or in a community outreach setting preferred
- Completion of Promotores de Salud Training highly desirable
- Bilingual English/Spanish highly desired
- Working knowledge of relevant social service programs and community resources
- Excellent communication skills with organization’s in target community
- Excellent active listening and conflict resolution skills
- Ability to relate to individuals and groups in an empathic and non-judgmental manner
- Excellent interpersonal, teamwork, detail-oriented, follow-through and verbal and written communication skills
- Willingness to serve persons with limited access to healthcare and from diverse cultures
- Proficient in data entry platforms such as Google Sheets, Excel, and various online services for seamless information management
- Ability to perform each essential duty and responsibility satisfactorily
- Comfortable analyzing reports and using data to drive performance results
- Must have reliable transportation
Physical demands:
- Ability to lift/move up to 35 pounds, for example files and promotional materials.
- Ability to participate in community health canvassing in the field, which involves accessing different work locations and travel throughout the assigned area and carrying program materials.
- Ability to travel for in-person meetings and events in South Los Angeles.
Compensation: Promotora de Salud/Community Health Worker is a full-time, non-exempt position with an hourly range of $20-25/hr.
