Community Health Worker (CHW) Apprentice – Chronic Disease Management & Diabetes Prevention

Program Overview:

This is a paid apprenticeship registered with the Maryland State Department of Labor through Heritage Care Inc.
Apprentices will gain hands-on experience while completing three nationally recognized certifications:
Certified Community Health Worker (CHW)
Diabetes Community Care Coordinator
Prevent T2 Lifestyle Coach

Training and Work Schedule:

Daytime (4 hours/day): Outreach and recruitment on behalf of Totally Linking Care (TLC) of Maryland
Evening (4 hours/day): Attend certification classes and training

Compensation:

$15/hour during CHW certification phase
$20/hour upon completion of CHW certification
$22/hour after completing Diabetes Community Care Coordinator and Prevent T2 Lifestyle Coach certifications

Key Responsibilities:

Conduct community outreach and client recruitment for TLC’s health programs
Promote the benefits of diabetes prevention and chronic disease management services
Attend and successfully complete all required certification classes
Provide support and care coordination to clients in underserved communities
Make home visits to deliver basic needs services and check on client well-being
Facilitate Prevent T2 diabetes prevention classes once certified
Assist in linking clients with medical providers, social services, and nutrition/fitness programs
Maintain accurate documentation of outreach, services provided, and client progress
Participate in case management team meetings and employer-sponsored professional development
