Overview:

Enoved & Co. is seeking a dedicated and compassionate PRP Counselor/Case Manager to join our team. This position offers flexible hours and the opportunity to make a positive impact in the lives of individuals accessing our Psychiatric Rehabilitation Program (PRP). Responsibilities:

Provide case management services to individuals enrolled in our PRP, including assessment, treatment planning, and coordination of services.
Serve as a liaison for the youth in the program with the school and probation officer
Facilitate skill-building groups and individual sessions to support clients in achieving their goals related to independent living, social skills, and community integration.
Collaborate with clients, families, and treatment teams to develop and implement individualized rehabilitation plans.
Assist clients in accessing community resources and support services to enhance their overall well-being and quality of life.
Maintain accurate and up-to-date documentation of client progress, treatment interventions, and service coordination activities.
Conduct comprehensive assessments of clients' needs, strengths, and goals
Coordinate and monitor services provided to clients, ensuring they are receiving appropriate care and support
Advocate for clients' rights and access to resources
Maintain accurate and up-to-date client records, documenting progress and outcomes
Stay informed about current research, best practices, and relevant regulations in the field of case management

Skills:

Bachelor’s Degree from an accredited university or college (or 30 credit hours toward degree) in a health-related field preferred.
Alternatively, an Associates Degree in a health-related field or one (1) year of work experience in a supervised mental health setting.
Strong interpersonal and communication skills, with the ability to establish rapport and build positive relationships with clients, families, and stakeholders.
Valid driver's license and clean driving record required.
Must be able to pass a background check.
Excellent communication skills, both verbal and written
Ability to work collaboratively as part of a multidisciplinary team
Strong organizational skills with the ability to prioritize tasks effectively
