Position Summary
The Social Work Case Assistant supports clients and their families throughout the process of accessing healthcare and community benefits. This role combines direct interaction with people and case management support: meeting with clients, understanding their needs, guiding them through applications, and maintaining accurate documentation and follow-ups.
Key Responsibilities
- Conduct meetings and conversations with clients and families to understand needs and provide guidance
- Monitor and track the progress of each client’s case from start to finish
- Assist with Medicaid applications, LTSS processes, and Appendix D forms
- Verify Medicaid eligibility and gather required documentation
- Support clients in navigating the Personal Care process and other Evergreen services (e.g., DMAS-225, UAI, PCG changes)
- Communicate regularly with counties, insurance companies, and state agencies to follow up on cases
- Help clients access additional low-income programs in Virginia (SNAP, energy assistance, housing, etc.)
- Educate clients/families about available services and explain next steps in clear, supportive language
- Keep all client records and applications updated and organized in the system
- Collaborate closely with caregivers, nurses, and office staff to ensure holistic support for each case
Qualifications
- High school diploma or equivalent (Associate’s degree or higher preferred; coursework in social work, human services, or related field is a plus)
- Previous experience in social services, healthcare, or community support strongly preferred
- Strong interpersonal and communication skills — ability to connect with clients and families with empathy and professionalism
- Organized and detail-oriented, with ability to monitor multiple cases at once
- Proficiency in Microsoft Office (Word, Excel, Outlook)
- Bilingual preferred — Bilingual highly preferred (Vietnamese, Korean, Spanish, Chinese) a plus
