Overview
The NOAH House Case Manager, working under the Court Services and Offender Supervision Agency (CSOSA) contract, is responsible for coordinating offender services, including medical, mental health, housing, and vocational supports, while providing individualized case planning and developing Individualized Living Plans (ILPs). This role includes participating in interdisciplinary team meetings, completing timely and compliant documentation, and developing aftercare and discharge plans to support successful reintegration. The Case Manager meets weekly with each assigned offender to monitor progress, address needs, and promote positive outcomes.
Responsibilities
- Conduct initial intake assessments to evaluate client history, needs, risks, and goals.
- Develop, implement, and maintain Individualized Living Plans (ILPs), ensuring plans are regularly updated based on client progress.
- Meet with assigned offenders on a weekly basis, or more frequently as needed, to monitor progress and provide support.
- Coordinate and connect clients to essential services, including medical care, mental health treatment, substance use services, housing resources, and vocational or job training programs.
- Maintain accurate, timely, and comprehensive case documentation, including progress notes, incident reports, and service tracking.
- Ensure all records meet agency, legal, and audit compliance standards; prepare files for internal and external reviews.
- Participate in interdisciplinary team meetings, providing updates on client progress and collaborating on service planning and interventions.
- Develop and implement discharge and aftercare plans prior to release, ensuring continuity of care through stable housing, ongoing treatment, and employment or training opportunities.
- Assist clients with completing applications and required documentation for benefits and services, including identification, Medicaid, SNAP, housing, and program enrollment.
- Provide referrals and guidance for community-based programs and support services; assist clients in navigating enrollment processes.
- Educate clients on program expectations, rules, and available resources.
- Monitor client participation and compliance with required programs; address behavioral concerns and non-compliance appropriately.
- Document and report incidents, risks, and significant changes in client status.
- Promote accountability and support positive behavioral change.
- Respond to urgent or crisis situations, including mental health concerns and interpersonal conflicts, utilizing de-escalation techniques, and connecting clients to emergency services when necessary.
- Perform additional duties as assigned by the Facility Director or designee.
Necessary Skills
- Ability to conduct comprehensive assessments, diagnose behavioral health conditions, and develop measurable, individualized treatment, and service plans.
- Ability to deliver evidence-based interventions and effectively manage crises, including de-escalation and safety planning.
- Demonstrated ability to coordinate clinical and supportive services and ensure continuity of care, including discharge and community reintegration planning.
- Ability to maintain accurate, timely, and legally sound documentation in compliance with all regulatory and ethical standards.
- Ability to engage diverse populations, collaborate with interdisciplinary teams, and effectively manage caseloads while promoting accountability and positive outcomes.
- High level of computer proficiency (Word, PowerPoint, Excel).
Pay: $55,000.00 - $65,000.00 per year
