Primary Duties:
- Supports client care coordination by conducting needs assessments, identifying eligible individuals, and facilitating access to appropriate health services and resources.
- Maintains accurate and timely documentation in EMR and tracking systems to monitor client interactions, services provided, and outcomes.
- Promotes health education, self-management, and quality improvement initiatives through client engagement, counseling, and support.
- Collaborates with internal teams, external partners, and community agencies to ensure continuity of care and address care gaps.
- Ensures compliance with organizational policies, safety standards, and regulatory requirements while actively participating in trainings, meetings, and grant-related activities.
Qualifications:
- High School Diploma or GED with 2 years of experience (Required)
- Bachelor’s degree (Required)
- Master’s degree in public health or related field (Preferred)
- 1-2 years in a healthcare environment. Strong understanding of cultural competency with target population
License/Certifications:
- No special certification, registration, or license (Required)
This position has a hiring range of : USD $28.76 - USD $48.96 /Hr.
