As a Social Work Assistant at Montcare Wheaton, you will play a vital role in supporting our mission to provide compassionate and individualized care to patients across our rehabilitation and skilled nursing centers. As a Social Work Assistant, you will work closely with our multidisciplinary teams to facilitate patient-centered care, promote independence, and ensure a seamless transition to the next level of care.
Responsibilities:
- Assist social workers and other healthcare professionals in assessing patient needs, developing individualized care plans, and implementing interventions to improve patient outcomes
- Provide emotional support and counseling to patients, families, and caregivers, addressing physical, emotional, and spiritual needs
- Facilitate patient education, discharge planning, and coordination of services with community resources
- Collaborate with healthcare teams to identify and address social, emotional, and environmental barriers to patient care
