POSITION SUMMARY
The Licensed Independent Clinical Social Worker (LICSW) is responsible for delivering advanced clinical social work services to residents in a Continuing Care Retirement Community (CCRC). This position involves conducting comprehensive assessments, developing and overseeing treatment plans, providing psychotherapy, and facilitating access to community resources. The LICSW plays a critical role in the interdisciplinary team, contributing to the overall mental health and well-being of residents. Strict adherence to confidentiality and privacy practices is required to protect residents' personal and health information. Success in this role hinges on recognizing the interconnected nature of operations and establishing and nurturing meaningful and effective relationships to strengthen the community.

Clinical Assessment and Treatment Planning:
- Perform in-depth psychosocial assessments to identify residents' mental health needs.
- Develop, implement, and monitor individualized treatment plans, adjusting as necessary based on ongoing evaluations.
- Provide clinical oversight and guidance to the interdisciplinary team on complex cases.
Psychotherapy and Counseling:
- Offer individual and group psychotherapy sessions to residents, addressing a range of mental health concerns such as depression, anxiety, grief, and adjustment disorders.
- Facilitate family counseling sessions to support residents and their families in coping with the emotional aspects of aging and transition.
- Ensure all therapeutic interactions are conducted with the highest level of confidentiality, in compliance with HIPAA and other relevant privacy regulations.
Interdisciplinary Collaboration and Coordination:
- Collaborate with nursing, therapy, and other departments to ensure a holistic approach to resident care.
- Participate in care conferences, interdisciplinary meetings, and discharge planning, providing expert input on mental health aspects.
- Coordinate with external mental health providers and community resources to enhance the continuum of care.
Documentation and Compliance:
- Maintain accurate and comprehensive clinical documentation in accordance with state, federal, and organizational standards.
- Ensure all social work practices comply with ethical standards and CCRC policies.
- Protect the confidentiality of resident records and communications, sharing information only when necessary and appropriate.
Crisis Intervention and Risk Management:
- Provide immediate crisis intervention for residents experiencing acute mental health issues.
- Develop and implement risk management strategies to address potential safety concerns, including suicide prevention and behavioral management.
Training and Education:
- Educate residents, families, and staff on mental health topics, therapeutic approaches, and wellness strategies.
- Lead staff training sessions on recognizing and responding to mental health issues and maintaining resident confidentiality.
- Attendance & Punctuality: Reliable attendance is essential. Staff must be punctual and dependable to meet community needs. Absences or tardiness disrupt work and burden colleagues. Attendance and reliability impact performance evaluation and continued employment.
QUALIFICATIONS and EXPERIENCE
Master’s degree in social work (MSW) from an accredited institution. Current LICSW license in the District of Columbia. Minimum of 5 years of clinical experience, preferably in a healthcare, geriatric, or long-term care setting. Advanced clinical assessment and psychotherapy skills. Strong communication and interpersonal abilities. Expertise in interdisciplinary collaboration and care coordination. Proficiency in electronic medical records (EMR) and clinical documentation. In-depth understanding of confidentiality requirements, including HIPAA regulations.
Working Conditions: Full-time position, Monday through Friday, with potential evening or weekend hours as required. Ability to function effectively in a dynamic and fast-paced environment. Occasional travel within the community or to external agencies may be necessary.
Physical Requirements: Ability to sit, stand, and walk for extended periods. Must be able to lift up to 25 pounds occasionally.
