GENERAL JOB DESCRIPTION

The role of the Rehabilitation Specialist is one of guidance, competence and support. This role is pivotal in creating the

Individualized Plan of Care for each client we serve. Additionally, a huge role is ensuring the competence of the Direct Care

Specialist to be able to implement the identified methods that would best support each individual client. Collaboration of both professional and natural supports for each client is also an important aspect of this role.

MAJOR DUTIES AND RESPONSIBILITES

Obtain authorization for PRP services

Initial contact is made within 48 hours of email notification.

Scheduling intakes should be reflected on the agency calendar

Document unsuccessful attempts on a contact note and list them in the weekly report

Reauthorizations should be completed 30 days before expiration of authorization

Meet with program director weekly to present updates, discuss staff concerns, program concerns, and regulatory concerns

Responsible for the discharge recommendation and summaries of clients

Prepares and monitors Individual Rehabilitation Plan (IRP) and maintains accurate, up-to-date records/documentation. Also provides addendum as needed.

Review progress notes to verify that direct care counselors are effectively addressing the treatment needs and implementing the goals and/or objectives set forth in the Individual Rehabilitation Plan (IRP).

Conducting weekly supervision with the counselors.

With guidance from supervisor, develops and implements rehabilitative on-site activities and groups that support related to consumer's needs based on IRP goals

Respond timely to emails and phone calls.

Scheduling collaboration meetings every 90 days with therapist, DCW, Specialst and any other member of the clients support team

Review & approve invoices. Only approve visits with completed documentation (Notes, Transition Summaries, Individual

Rehabilitation Plans (IRPs), Monthly Summaries, Memo Section). Documentation is considered complete when it has been inputted and signed in the Electronic Health Records system and IRP '_s have been signed by client and placed in supervisor '_s box.

Review weekly billing summary - match billing summary with documentation submitted in Electronic Health Records system and follow up to ensure documentation is current including memo section

Coordinates comprehensive care to assigned clients.

Maintains accurate, up-to-date documentation of all services in a timely manner. In addition to contact notes, end-of-day notes, and incident reports, is responsible for monthly progress notes, IRPs and utilization and documentation of appropriate assessment tools (DLA-20s), as directed by supervisor.

Coordination of services with consumer's other providers (psychiatrist, therapist, etc.) by effectively and professionally communicating clinical matters on a monthly basis.

Identifying consumer's needs and linking consumer with community services.

MINOR DUTIES AND RESPONSIBILITIES

Posting Daily Schedule on InSync and/or Share Point

Attending Weekly Clinical Meetings every Friday Morning

Update Resource Binder as necessary and appropriate

Identify monthly events in the community for both our adult and adolescent clients

Identify community resource opportunities for all clients in the program
QUALIFICATIONS FOR THE JOB

Education:
LGPC
LCPC
LMSW
LCSW

Experience:

At least one year working with individuals in marginalized communities. At least two years of experience working with children and families.
