We are a nonprofit, community-based organization providing Housing and Supportive Services to adults with mental illness, SUD and other co-occurring disorders.

We are looking for a full-time Housing Stabilization Case Managers serving Northern Virginia.

YOU WILL:

Have a Monday-Friday schedule that is convenient and flexible. No weekends or holidays.

Work within a supportive environment meeting regularly with your supervisor and teammates.

WHAT WILL YOU BE DOING?

You will implement our successful model for recovery by:

supporting individuals in successfully transitioning into Permanent Supported Housing from state hospitals, other supervised residences, or homelessness without disruption in their psychiatric stability or sobriety.
assisting individuals with independent living, sobriety, psychiatric stability, housing stability and engagement in the community.
supporting individuals’ ability to remain in housing by assisting them with adhering to lease terms, ensuring compliance with fire and safety standards , and developing positive relationships with neighbors, landlords and the community.
advocating for, and supporting individuals in self-advocacy related to leasing, fair housing and reasonable accommodations.
coordination of care for assigned individuals to include assistance with completing referrals to needed services and supporting the individual’s access to available community resources.

Required qualifications: Bachelor’s degree in social work, counseling, psychology or related field. Minimum one year experience working with individuals with co-occurring mental health and substance use disorders, and/or experiencing homelessness. Must be able to pass a driving records check and a criminal background check.

Compensation is $50,024 annually.
