Overview

We are seeking a dedicated and highly skilled Registered Nurse Case Manager to join our healthcare team. In this vital role, you will coordinate patient care across various settings, ensuring seamless transitions from inpatient to outpatient services and post-acute care. Your expertise in diverse clinical environments will support optimal patient outcomes, efficient utilization of resources, and comprehensive discharge planning. The ideal candidate will possess extensive nursing experience, including critical care, trauma, emergency medicine, and specialized fields such as pediatrics, geriatrics, and behavioral health. Duties

Conduct thorough patient assessments, including vital signs and physiological evaluations, to determine appropriate care plans.
Develop and implement individualized discharge plans that incorporate post-acute care options and community resources.
Collaborate with multidisciplinary teams to coordinate inpatient, outpatient, and clinic-based services ensuring continuity of care.
Manage utilization review processes by evaluating diagnostic evaluations and medical necessity for hospital stays and procedures.
Facilitate effective communication with patients, families, healthcare providers, and insurance companies while maintaining HIPAA compliance.
Monitor patient progress through medical records review and ongoing assessment to adjust care plans as needed.
Lead case management activities across diverse clinical settings such as ICU, PICU (Pediatric Intensive Care Unit), ER (Emergency Room), Geriatrics units, and behavioral health services.
Oversee intake procedures for new patients requiring complex medical-surgical or acute care services.
Support hospital experience initiatives by ensuring timely documentation of medical records and discharge summaries.
Participate in quality improvement initiatives related to infection control, trauma care protocols, and emergency medicine standards.

Skills

Proven experience in critical care environments such as ICU, Level I Trauma Centers, or Level II Trauma Centers.
Strong background in hospital medicine, emergency medicine, or medical-surgical nursing with a focus on acute care.
Expertise in managing complex cases involving infectious disease care, geriatrics, pediatrics (including PICU), and behavioral health populations.
In-depth knowledge of physiology principles relevant to patient assessment and diagnostic evaluation.
Familiarity with managed care processes including utilization management and outpatient clinic coordination.
Proficiency in medical terminology, medical records management, and discharge planning procedures.
Excellent communication skills for effective patient service delivery and interdisciplinary collaboration.
Ability to navigate healthcare technology systems for documentation and case management purposes.
Knowledge of HIPAA regulations to ensure confidentiality and privacy of protected health information.
