JOB SUMMARY

Metro Day is looking for an Activities Coordinator who will be responsible for assisting the Program Director in planning, scheduling, and coordinating meaningful activities appropriate to the needs and interests of the Persons supported. The activities Coordinator will help to ensure the programs are appropriately presented by staff.

JOB DESCRIPTION

Prepare planning documents required by regulation, MDTCI policy/practices based on assessments and information submitted by members of the Interdisciplinary Team (IDT).
Documents include, but not limited to the ISP’s; BSP, and other clinical assessments; meeting minutes and periodic reports and other documents deemed necessary to meet regular requirements on the needs of people served. Print, review and ensure implementation of the ISP’s for people assigned to their caseloads.
Monitor, train, and implement individual service plans for people assigned to the caseload.
Inform the supervising Program Director of issues identified as problematic relative to the implementation of the plan to include, but not limited to the following: Behavior Management; Health Care Management Plan; Nutritional Services, Day Program Services,
Maintain communication with residential provider and DDS Service Coordinator as needed to ensure team members are aware of identifies issues. Facilitate planning with the team to resolve issues and modify the Persons Program Plan (IPP) accordingly.
Conduct/completion of Functional Assessment in conjunction with the Treatment Team.
Prioritize identified needs and establish programs for those needs that are prioritized.
Facilitate the acquisition and or development of necessary supports required by people in the caseload to achieve expectations identified in the ISP.
Distribute quarterly comprehensive reports as required by MDTCI and Procedures.
Assume other responsibilities as assigned by the supervising Program Director not limited to the acquisition/development of necessary report and support required by the people in their caseload.

QUALIFICATIONS

Bachelor’s Degree in a Human Services or Related Sciences field, with a minimum of one (1) year experience with persons with developmental disabilities.
Master’s Level Licensed Nurse (MSN), Social Worker, Licensed Clinical (LCSW), Masters Level Professional Counselor (LPC), or master’s Level Psychologist, with a minimum of one (1) years’ experience in the case management for person with developmental disabilities: or
Experience in staff supervision
Valid driver’s license.

Job Type: Full-time

Pay: $55,000.00 - $60,000.00 per year
