The RN Case Manager position is responsible for working to meet the physical, medical, psychosocial, emotional and spiritual needs of hospice patients and families facing terminal illness and bereavement. They will meet with the patients at their home to provide care. The RN Home Care nurse will work directly with the patient. The total amount of visits may vary per day. This position requires a medical license.

Qualifications
Candidates for the Registered Nurse Case Manager position should possess core skills such as managed care, patient service, and nursing, along with premium skills like critical care experience and ICU experience. Relevant skills in inpatient and outpatient care are also beneficial. A valid RN license is a requirement for this role, ensuring the ability to provide quality patient care and support.

Pay: $44.00 - $46.00 per hour
