We are seeking a dedicated and energetic PRP (Pychratric Rehabilitation Program) Case Manager to join our dynamic healthcare team. In this vital role, you will coordinate comprehensive case management services for patients across outpatient care settings. Your expertise will help ensure seamless patient transitions, optimize recovery outcomes, provide resources, and promote efficient utilization of healthcare resources. Duties

Conduct thorough patient concerns/needs, including medical history review, physiological evaluation, and social determinants of health to develop personalized care plans.
Coordinate discharge planning and post-acute care arrangements to facilitate smooth transitions from hospital to home or other care facilities.
Collaborate with multidisciplinary teams—including social workers, behavioral health specialists, addiction counselors, and medical staff—to ensure comprehensive patient support.
Facilitate intake procedures for new patients, ensuring accurate documentation and timely initiation of case management services.
Oversee outpatient case management activities.
Provide education on medical terminology, physiology concepts, and discharge instructions to patients and their families to promote understanding and adherence to treatment plans.

Requirements

Proven experience in Maryland Resources
Strong background in hospital medicine, behavioral health, mental health counseling, or social work with a focus on patient service excellence.
Familiarity with HIPAA compliance standards and medical records management.
Experience in utilization management, discharge planning, post-acute care coordination, and managed care environments.
Excellent communication skills for effective collaboration with multidisciplinary teams across hospital and clinic settings.
Great documentation timely, time management, communication skills, and other assigned duties.
