Position Summary

The Care Coordinator develops service plans and coordinates the care of assigned clients.

Principal Duties and Required Skills

Coordinate client admissions
Make ongoing clinical decisions for assigned clients in collaboration with the treatment team and owners
Coordinate the medical care of assigned clients
Complete client documentation accurately and on time
Provide overall care planning and guidance for clients living with severe mental illness

Minimum Requirements

Master's degree in a mental health–related field with a minimum of two years of related experience, OR a Bachelor's degree in a mental health field with a minimum of five years of mental health–related experience
Experience in a psychiatric rehabilitation or clinical care setting working with clients who have severe and persistent mental illness
Valid driver's license with a satisfactory driving record; must have access to a personal vehicle and may be required to transport clients

Schedule

This is a full-time, in-person position with core hours of Monday–Friday, 9:00 AM–5:00 PM
Occasional flexibility in start/end times may be available based on operational needs, but candidates must be consistently available during core business hours
This position is not remote and does not offer a compressed or alternative workweek schedule

Physical Demands

Must be able to move efficiently throughout the day, including stairs, and stand and/or sit for extended periods
Must be able to lift 10–30 pounds as needed
