The Case Manager works under the supervision of the Facility Assistant Director.
Parameters of the Position:
1. Case Management -works within the rehabilitative framework designed by the Facility Director, that includes, but is not limited to:
a. individual and group counseling;
b. specialized habilitation methods;
c. use of community & employment resources; and
d. resident management, accountability and control;
The incumbent uses counseling skills and therapeutic knowledge to establish the necessary treatment and case management practices for individual returning citizens.
Duties and Responsibilities
The Case Manager is responsible for:
· Carrying a caseload of up to fifteen (15) returning citizens/clients, or based on DOC Contract
· Conducting the returning citizens’/clients’ orientation to the program and assuring their understanding for compliance with the rules and expectations of the program on a continuous basis;
· Completing an assessment tool to prepare program plans for re-entry;
· Utilizing individual counseling to develop resident release plans and monitoring the development of that plan;
· Keeping accurate and timely case notes, preparing case reports and tracking client activity and progress data;
· Effectively managing resident schedules to include printing and distribution, as required by the contract source.
· Assisting in the planning, developing, implementing, monitoring, accountability and leading of resident and operational programs;
- Conducting Life Skills Enhancement classes on a weekly basis;
· Performing duties related to disciplinary procedures as directed;
· Preparing and submitting reports to the prescribed criminal justice agencies or judiciary with the approval of the Director;
· Initiating and maintaining ongoing personal contacts with a variety of business and industry representatives, social service agencies and job placement/training agencies to promote programs for returning citizen/client placement;
· Explaining the benefits and employment support services provided by programs to employers, including addressing employer’s special needs;
· Familiarizing themselves with procedures for eligibility placement of returning citizens/clients in subsidized employment programs;
· Researching the internet, newspapers, agencies, and other resources for job leads;
· Locating jobs for returning citizens/clients; matches job skills with returning citizens’/clients’ qualifications; refers qualified returning citizens/clients to employers and conducts necessary follow-up, when returning citizens/clients are placed in positions;
· Keeping an updated information regarding job fairs and internet resources; researches various resume programs;
· Participating in outreach and recruitment activities, by coordinating and attending job fairs.
· Driving returning citizens to interviews, job fairs, and other organizations;
· Participating in in-service and other training programs that enhance counseling, case management, life/safety, and administrative skills;
· Interpreting the agency policy on an as-needed basis;
· Collecting resource, demographic, and research data as directed;
· Conducting field and home contacts with returning citizens/clients, their families, and their employers up to one hundred (100) miles from the facility;
· Testifying in court when so directed;
· Getting familiarized with the rules, regulations and the ability to make decisions in the best interest of the returning citizens/clients.
Learning to effectively utilize the SecurManage system to maintain electronic records, i.e. case notes, assessments program plans, termination reports, etc.;
· Performing other duties as assigned.
Education and Qualification
The CM must possess a Bachelor’s degree in criminal justice, sociology, social work, psychology, or a related field profession from an accredited college or university. At a minimum, at least two (2) years of relevant human services experience is required.
Work experience may be substituted for academic studies, exchanging one year of work experience in a related field, for one year of academic education. Must be familiar with the area surrounding the facility, and willing to travel up to one hundred (100) miles, each way from the office. Must be highly skilled in Microsoft Word and Excel and display
the ability to format and create documents, reports and charts independently. Must have a valid driver’s license, a good driving record and a reliable vehicle.
Clearance
Employees assigned to this position, must satisfy all requirements under the contracts with applicable federal state and local government agencies. All employees must pass an NCIC criminal record background check, after conditional offer of employment. Each employee will be fingerprinted by the Bureau of Prisons staff and/or a designee, and is subject to investigation.
