Summary/Objective
The Licensed Graduate Social Worker provides vital psychosocial care and support to clients and their families under the supervision of an independently licensed clinician (LICSW, LPC, or Licensed Psychologist). This role includes conducting diagnostic assessments, counseling, case management, and coordination of care with healthcare professionals to address clients’ emotional, social, and mental health needs in home health and community settings. The Dependent Social Worker collaborates closely with the interdisciplinary team while ensuring the highest quality of care.
Essential Functions
- Conduct initial biopsychosocial assessments under the supervision of an independently licensed clinician.
- Assist in developing and executing treatment plans addressing the mental, social, and emotional needs of clients.
- Provide case management services, coordinating with community resources and healthcare providers to ensure seamless client care.
- Collaborate with nurses, physicians, and other healthcare professionals in multidisciplinary team meetings to plan client care strategies.
- Participate in client and family counseling sessions under the guidance of a licensed clinical social worker.
- Collaboratively document client interactions and care plans in the electronic medical record (EMR) system.
- Facilitate client access to social services and community resources, ensuring they receive necessary support.
- Assist in organizing and leading client and family support groups as required.
- Ensure compliance with HIPAA regulations and organizational policies to maintain client confidentiality.

- Ability to manage multiple cases and prioritize tasks effectively.
- Cultural competence and sensitivity when working with diverse populations.
- Basic case management and client advocacy skills to support clients' rights and access to services.
Physical Demands
The position requires prolonged periods of sitting, standing, and movement. Occasionally, lifting (up to 20 pounds) may be necessary.
Position Type and Expected Hours of Work
Part-Time position, Required to be on site Mon-Fri 3pm-6:30pm in Southwest DC (flexible on arrival if needed please communicate if chosen for an interview)
Travel
Regular travel within the Washington, DC metro area for client visits and community meetings is expected.
Required Education and Experience
- Master’s degree in social work (MSW) or a related human services field from an accredited institution.
- Minimum of 1-2 years of experience in a social work or human services role, preferably in healthcare or behavioral health settings.
- District of Columbia Graduate Social Work License (LGSW) is required.
Preferred Education and Experience
- Experience with diverse populations, such as pediatric or geriatric clients.
- Multilingual abilities (e.g., Spanish) are preferred but not required.
Additional Eligibility Qualifications
- Current CPR certification.
- Valid driver’s license and reliable transportation for home visits.
