Capitol City Rehab and Healthcare Center is a 360-bed skilled nursing center conveniently located in Washington DC. We provide long term nursing care, as well as short term rehabilitation in our designated rehabilitation wing.
Responsibilities:
- The social worker will interview residents and families to obtain social history
- The social worker will maintain written documentation in the resident medical record per the company's policy.
- The social worker will develop social service component of the resident's plan of care identifying specific problems, goals, and approaches.
- The social worker responsibilities involve residents and families in planning social service programs when possible
- The social worker will collaborate with interdisciplinary team daily
- The social worker will interpret social, psychological, and emotional needs of the resident, and family to the medical staff, attending physician, and other resident care team members.
- The social worker will provide solutions for social and practical environmental problems including seeking financial assistance, discharge planning, collaboration and referrals with community agencies as needed
The successful Social Worker Candidate:
- Licensed as an LMSW in Washington DC.
- The Licensed Social Worker has previous discharge planning experience and knowledge of community-based programs offered
- preferably has experience in a long-term care setting.
- excellent communication and interpersonal skills.
