Job Overview
We are seeking a dedicated and compassionate Case Manager to join our healthcare team. The ideal candidate will have a strong background in patient care and a thorough understanding of medical terminology, case management, and discharge planning. As a Case Manager, you will play a crucial role in coordinating patient services, ensuring that individuals receive the appropriate care and support throughout their treatment journey. Responsibilities

Conduct comprehensive patient assessments to determine needs and develop individualized care plans.
Collaborate with multidisciplinary teams to facilitate effective discharge planning and post-acute care transitions.
Manage cases across various settings including inpatient, outpatient, and clinic environments.
Provide support for patients dealing with behavioral health issues, addiction counseling, and mental health challenges.
Ensure compliance with HIPAA regulations while maintaining accurate medical records.
Utilize knowledge of medical terminology and physiology to educate patients about their conditions and treatment options.
Perform diagnostic evaluations and assist in utilization management processes.
Advocate for patients' rights and coordinate resources to enhance their overall well-being.

Skills

Hospital experience is essential; Level I or Level II trauma center experience is highly desirable.
Familiarity with acute care settings including ICU, PICU, emergency medicine, and hospital medicine.
Experience in social work or behavioral health is beneficial for addressing complex patient needs.
Strong understanding of patient service protocols, patient assessment techniques, and discharge planning processes.
Knowledge of medical records management and intake procedures is crucial for effective case management.
Experience in outpatient settings or clinic environments can enhance the quality of care provided.

Job Type: Contract

Pay: $22.62 - $30.00 per hour
