Job Title: Licensed Independent Clinical Social Worker (LICSW) - Care Manager
Location: Washington, DC (20032)
Schedule: Monday - Friday, 8:00 AM - 5:00 PM
Pay Range: $43.00 - $47.00/hour (based on experience)
We are seeking experienced and compassionate Care Managers (LICSWs) to provide case management services for enrollees in the DC Medicaid program. Many members face behavioral health challenges and complex social determinants of health. Care Managers play a critical role in conducting assessments, developing care plans, and coordinating services to support enrollees and their families.
Conduct needs assessments to determine enrollee status and assign an acuity level (1-3):
Level 1: Well-managed, require minimal Medicaid resources.
Level 2: Moderate needs, requiring additional attention.
Level 3: High utilizers with complex needs, requiring frequent touchpoints.
Develop and update care plans based on assessment findings.
Perform approximately 3 care plans/assessments per week, which include field visits (about 3 days/week in the field, 2 days for documentation).
Collaborate with families, providers, and community resources to reduce barriers and improve health outcomes.
Attend well-child visits when appropriate to provide support and update care plans.
Build relationships with primary care providers to ensure coordinated and comprehensive care.
Standard LICSW: Caseload of 75 patients (mix of acuity levels 1-3).
Specialty LICSW: Caseload of 60 patients (primarily level 3 with complex needs).
Initial caseload begins at ~30 patients and gradually increases to full capacity.
3-week onboarding program including orientation, documentation training, and Medicaid contractual guidelines.
Combination of in-office and remote work during training; more in-office support during the first 3 months.
Weekly in-person team meetings (Tuesdays) for collaboration and mentorship.
Assigned preceptor support selected by supervisors.
$43-44/hour for 1-2 years of experience
$45/hour for 3-4 years of experience
$46/hour for 5 years of experience
$47/hour for 6-7 years of experience
Meaningful work serving underserved populations.
Somewhat flexible scheduling for field visits.
