COMPENSATION
· Full Time Salary $70,000 to $95,000 depending on experience. Part Time $40-50+/hour depending on experience.

ESSENTIAL JOB FUNCTIONS:
The successful candidate must be willing, helpful, supportive, cooperative, accountable, have attention to detail and be able to make decisions based on established guidelines and procedures. The essential functions of this position are people focused and helpful in nature; providing support and/or service to others. Care managers will work with adult/aging clients who need unbiased and professional assistance. This includes guidance related to healthcare, aging, household & financial tasks. Responsibilities include but are not limited to:
- Assess and evaluate and assess older adults and adults living with chronic illnesses.
- Develops comprehensive care plans to address client needs and goals.
- Conducts routine home and community visits.
- Transports clients to medical appointments and outside outings. Personal vehicle is required and utilized.
- Advocates for clients.
- Provide education to clients/families about options for support resources.
- Attend bimonthly in person staff meetings and company trainings as required.
- On-Call Coverage: Able and willing to participate in a rotating after-hours on-call schedule.
EDUCATIONAL/EXPERIENCE AND REQUIREMENTS:
- Associate’s, Bachelor’s, or Master’s degree in a healthcare related field, such as social work, nursing, counseling, gerontology, mental health, physical therapy, occupational therapy, or other related health field.
- Previous experience working in the health care field or with older adults/adults with chronic illnesses and their families/caregivers preferred.
- Care management with older and/or disabled adults preferred.
- Experience working with individuals with dementia preferred.
- Must have some knowledge of community resources, entitlements, and private insurance systems.
- Reliable vehicle, active driver’s license, and safe driving record.
- Knowledge of Microsoft Office Suite (excel, word, outlook, power point).
- Availability and willingness to take on-call rotation.
JOB SKILLS & ABILITIES INCLUDE:
- Excellent written and verbal communication skills with attention to detail.
- Ability and willingness to travel throughout the DMV area during work hours as dictated by position needs.
- Ability to work independently and collaboratively with interdisciplinary teams.
- Proficiency with technology, including use of smartphone and web-based office systems.
- Ability to work independently, organize travel efficiently, manage multiple projects in a fast-paced environment.
- Ability to pass a background screen.
MENTAL AND PHYSICAL REQUIREMENTS
- The team member regularly is required to stand, walk, sit, bend and reach with hands and arms above, below and at shoulder height.
- The team member frequently lifts and/or moves up to 25-50 pounds.
- Must be able to push a wheelchair a minimum of 250 pounds and be able to push such weight a minimum distance of 300 feet without stopping.
- Work area may be subject to noise, distractions, and interruptions. Ability to remain calm and positive in stressful situations is required.
- To be eligible, applicants must reside in Northern Virginia.
