Treatment Foster Care Clinical Case Manager
Salary 55k – 60k, dependent upon licensure and experience
Essential Duties and Responsibilities
- Manage an average caseload of eight youth
- Conduct bi-monthly home visits to youth residing in treatment foster homes to ensure medical, educational, and therapeutic needs are met
- Provide clinical support to treatment foster parents
- Serves as clinical lead for youth’s treatment team
- Facilitate youth’s treatment team meetings in collaboration with referral agency, foster parent, biological family, and service providers
- Develop initial and quarterly treatment plans
- Complete documentation in Electronic Health Record
- Ensure service provision and documentation is in compliance with COMAR regulations
- Maintains regular communication with referral source (DSS/DJS)
- Other relevant duties as assigned
Position Requirements:
- Maryland Licensed Social Worker (LBSW or higher)
- Valid driver’s license
- Experience providing services to youth in the foster care and juvenile justice system
- Ability to communicate clearly and concisely
