Case Manager- Coordinator of Community Services & Support Planner
(This is a post for two separate case manager positions)

Work Schedule: 8:30am-5:00pm, Monday through Friday (Flexible as needed)
Work Environment: Hybrid positions-Both positions work from home office*with local community visits*.

Position 1: Case Manager-Support Planner (SP)
The Supports Planner is responsible for identifying, coordinating, and monitoring services that support individuals’ health, safety, independence, and quality of life. This role includes waiver eligibility determination and ongoing maintenance. Supports Planners are mobile, community-based professionals with strong knowledge of local resources in their assigned service areas.
Support Planner compensation and bonus: Base salary $60,000 with a bonus potential of up to $5,000.
As a Support Planner you will,
- Build and maintain person-centered relationships with clients
- Advocate for client needs, rights, and preferences
- Provide education on available community services and supports
- Assist clients in understanding service options
- Develop and maintain individualized plans of service
- Support clients in achieving personal and life goals
- Identify and connect clients with qualified providers
- Coordinate and monitor service delivery
- Resolve service-related issues and concerns
- Maintain waiver eligibility and required documentation
Position 2: Case Manager – Coordinator of Community Services (CCS)
The CCS works directly with individuals with intellectual and developmental disabilities and their families to assess needs, coordinate services, and connect clients to community-based resources. This role focuses on service coordination and monitoring rather than waiver determination.
Coordinator of Community Services: Base salary of $65,000 with a bonus potential of up to $5,000
As a CCS you will,
- Collaborate with clients and families to identify needs, goals, and preferences
- Provide information on community resources and support services
- Support clients in making informed decisions and planning for the future
- Assist clients in accessing service delivery systems and supports
- Coordinate and monitor services to ensure alignment with client goals
- Evaluate service effectiveness and address concerns
- Maintain accurate documentation and compliance with program standards
Requirements for both positions (SP and CCS):
- Education Requirement: Bachelor’s degree in human services (e.g., social work, psychology, criminal justice, or a related field) from an accredited U.S. college or university, or a foreign degree with an educational evaluation demonstrating U.S. equivalency from an accredited evaluation service.
- Supports Planning some case management experience.
- Coordinator of Community Services, a minimum of 2 years of case management experience.
- Valid driver’s license, reliable transportation, valid auto insurance, and safe driving record.
- Strong communication, organization, and time-management skills.
- Must be 21 years of age.
- Must be legally authorized to work in the U.S.
