Job Description
Job Title: Health Educator and Navigator
Department: Community Health Action
Supervisor: Health Educator and Navigator Team Lead
Location: This position will be primary located in Hyattsville, Maryland; but need to be able to move between locations as needed. This position could work remotely, as needed.
Classification: Non-Exempt/Hourly
Synopsis: This position will develop and implement health education/ promotion initiatives with a focus on mitigating barriers to healthy behavioral change and facilitating access to social support services. The selected candidate will work with direct service providers to link patients with an array of education and health promotion services focused on chronic disease management and prevention including but not limited to weight reduction, improving nutritional literacy, promoting active lifestyles, wellness, and stress management. The selected candidate will also coordinate a navigation system to connect community members to existing local services and address the social determinants of health affecting patients.
Qualifications:
Required Education and Experience:
- Associates degree in a health field, or related discipline, or an equivalent combination of a High School diploma with a minimum of two years of documented experience in public health program implementation, or similar field.
- Bicultural; fully bilingual.
- Comprehensive awareness and knowledge of social determinants of health affecting the Latino immigrant community in the Washington DC metro area.
- Two years previous work experience in community health education and/or clinical services.
- Two years of experience with program data reporting.
- Experience with delivery of charlas.
- Experience working in a Latino and/or multi-ethnic communities.
Preferred Education and Experience:
- Experience working with community health workers.
- Experience implementing community educational campaigns, including the design of educational materials.
Required Skills/Abilities/Certifications/Licenses:
- Good oral and written communication skills in both English and Spanish.
- Ability to work with all segments of the community.
- Strong ethical core.
- Computer literate in Microsoft Office (Word, Power Point, Outlook, Excel).
- Ability to work with electronic medical records (EMR) or another database software.
- Leadership, interpersonal and problem-solving skills, detail oriented, teamwork-oriented, self-starter, highly organized.
Preferred Skills/Abilities/Certifications/Licenses:
- Experience using eCW.
- DPP Lifestyle Coach Certification
- DC, MD, or VA Community Health Worker Certification.
Duties and Responsibilities:
- Support direct service providers to identify patients and community members eligible for education/ support groups, system navigations around the areas of Access to Healthy Food, Access to Infrastructure for recreation and physical activity.
- Implement evidence-based curriculums for chronic disease prevention and management.
- Conduct on-going monitoring and reassessment of client progress and needs.
- Maintain medical records and other data bases including completeness, filing, neatness, confidentiality, and documentation.
- Conduct “charlas” (educational sessions), as needed.
- Support the production of culturally appropriate audiovisual education and informational materials and disseminate them within La Clinica.
- Participate regularly in medical and patient support services team meetings.
- Assist in preparing reports about health education as required by funders.
- Implement a strategy to recruit, engage and navigate health promotion clients:
a. Develop and maintain a referral mechanism with community-based organizations, to navigate Spanish-speaking immigrants who are living with chronic conditions to social support services.
b. Provide navigation services to following La Clinica’s guidelines and procedures.
- Other duties and responsibilities:
a. Assist supervisor in maintaining program inventory.
b. Attend required staff training to improve skills.
c. Attend program, department, and all staff meetings and all mandatory La Clinica activities.
d. Meet regularly with supervisor to report and review work plan.
e. Represent La Clinica by attending conferences, meetings, coalitions, and community planning groups meetings, as designated by supervisor.
f. Follow La Clinica’s Code of Conduct, and all other personnel Policies and Procedures.
g. Perform other related duties as assigned.
Supervisory Responsibilities:
None.
Physical Requirements:
- Prolonged periods of sitting at a desk and working on a computer.
- Must be able to lift up-to 15 pounds at times.
- Must be able to work outside and stand-up for a prolonged period during outreach events or outdoor group sessions.
- Ability to work weekends and evenings, as needed.
- Able to perform duties in different La Clinica’s locations as required.
- Able to perform duties and responsibilities remotely when needed.
