Position Summary
The Licensed Clinical Social Worker (LCSW-C) provides assessment and case management services to seniors and adults with disabilities participating in Montgomery County’s Social Services to Adults and Home Care programs.
This role supports clients by assessing needs, coordinating services, facilitating community resources, and maintaining documentation to ensure continuity of care and support services.
The position includes both community-based visits and in-office screening services.
Primary Duties
- Conduct assessments and provide ongoing case management services for eligible seniors and adults with disabilities.
- Visit clients in community settings including private homes, group homes, and nursing facilities.
- Provide community resource referrals and facilitate service linkages.
- Document all client interactions and case activities within the electronic case management system.
- Conduct in-office screenings and phone assessments for program eligibility.
- Provide assistance to walk-in clients seeking services.
- Perform financial eligibility reviews when required.
Minimum Qualifications
- Licensed Clinical Social Worker – LCSW-C (Maryland State Board of Social Work Examiners)
- Experience conducting screening and assessments
- Experience providing case management services
- Experience writing psychosocial summaries
- Excellent writing and communication skills
- Ability to maintain documentation in electronic case management systems
Preferred Qualifications
- Prior experience working with seniors and adults with disabilities
- Knowledge of local community resources and service providers
