Case Manager

TANF Education Occupational Training (EOT) and Job Placement (JP) Services

Summary: JHP seeks a Case Manager to assist Customers through intensive, holistic case management to assist in guiding them through career development and educational pathways that result in gainful employment and self-sufficiency.

Position Responsibilities:

Under the direct supervision of the Program Manager, the Case Manager will:

Manage a portfolio of Customers seeking employment by motivating their education and job search efforts, which include coaching through barrier removal, professional development, and job placement
Develop and manage the comprehensive working relationship with all stakeholders including customers, governmental agencies, employment partners, service providers, etc.
Conduct escalating outreach to customers who fail to report to mandatory appointments and services
Identify and assist with mitigating barriers to program participation, including but not necessarily limited to, health, mental health, housing, childcare, domestic violence issues
Re-engage clients by discussing the range of available services and the potential benefits
Maintain updated customer database and track customer engagement in services
Accurately document case management activities in the State database daily
Coach and mentor job seekers through personal and professional development
Prepare reports of activities and achievements
Other duties and projects, as assigned

Qualifications:

· A minimum of a bachelor’s degree in social work, social services, counseling, business administration or related field from an accredited college or university or a minimum of five (5) years’ experience providing workforce development-related case management services to populations like the District’s Customers with a professional certification

· Worked with Customers in a caring and respectful manner with an understanding of and consideration for their unique circumstances

· Prior experience developing, evaluating and implementing a service plan that is milestone driven; experience in identifying community resources/services and connecting Customers to resources/services

· Able to submit written forms and reports accurately

· Good at building trusting relationships

· Effective communicator

· Good listening and coaching skills

· Computer literate and proficient at using Microsoft Office

· Other duties and projects, as assigned

Salary and Benefits Package:

· $48,000 - $50,000 DOE
