Position Summary
As a Care Support Worker at Universal Health & Serenity Pathways, you will play a key role in supporting clients enrolled in our MHRS and SUD programs. You will work directly with clients, their families, and our clinical team to ensure that each individual’s treatment plan is implemented effectively, progress is monitored, and barriers to recovery are addressed. You’ll support clients from intake through stabilization and after-care to promote sustained recovery.
Key Responsibilities
- Conduct outreach and initial engagement with clients enrolled in MHRS and SUD programs.
- Assist clients with intake processes, including gathering client history, verifying insurance/coverage, scheduling initial appointments.
- Monitor clients’ progress and adherence to treatment plan goals (including therapy sessions, group attendance, medication compliance where applicable).
- Identify barriers to progress (transportation, housing, social supports, mental health comorbidities) and coordinate with clinical staff or external agencies to address them.
- Facilitate referrals to internal and external resources (housing, employment support, peer support groups, case management).
- Maintain accurate records in accordance with clinic policy, regulatory requirements, and confidentiality standards (e.g., HIPAA).
- Communicate with clients’ families (with permission) and provide updates regarding progress, needs, and available supports.
- Assist with discharge planning and after-care coordination to promote continuity of care and prevent relapse.
- Participate in team meetings, case reviews, and contribute to improving client support workflows and service delivery.
Qualifications
- Bachelor’s degree in Social Work, Psychology, Counseling, Human Services, or a related field; OR an Associate’s degree plus relevant experience in mental health/SUD work.
- Experience working with individuals with mental health disorders and/or substance use disorders preferred.
- Excellent interpersonal communication skills; ability to build rapport with clients, families, and multidisciplinary team members.
- Strong organizational and follow-up skills; ability to manage multiple clients/cases concurrently.
- Familiarity with community resources in Washington, DC (housing, employment services, peer support) is a plus.
- Proficient with electronic health records, documentation, and confidentiality practices (HIPAA).
- Ability to work flexible hours (some evenings or weekends may be required) depending on client needs.
- Valid driver’s license and reliable transportation may be required for off-site visits or community outreach (if applicable).

Pay: $23.42 - $28.20 per hour
