Job Summary

Hospital Support Services (HSS) is seeking a Case Manager – Medicaid Eligibility to manage healthcare-related Medicaid (Medical Assistance) cases for hospital-referred patients. This position is responsible for eligibility determination, case management, patient outreach, documentation, and coordination with hospitals and social services agencies. Accuracy, organization, and timely follow-up are critical to success in this role.

Core Responsibilities

Manage a caseload of Medicaid eligibility cases from intake through approval
Perform eligibility screening and determine appropriate Medicaid category
Enter, update, and maintain accurate case management records in the HSS database
Communicate with patients to collect required eligibility documentation
Coordinate with hospitals, social services agencies, and field staff
Request, review, and submit medical records as part of the eligibility process
Track case timelines and follow up to ensure timely Medicaid enrollment
Communicate with Department of Social Services caseworkers to obtain approval
Review completed cases for accuracy, compliance, and payment processing
Ensure all healthcare eligibility documentation is complete and current
Perform other case management duties as assigned

Required Qualifications

High school diploma or equivalent work experience
Experience in healthcare administration, case management, eligibility, or social services preferred
Strong data entry and computer skills
Excellent organizational and time-management abilities
Clear written and verbal communication skills
Ability to manage multiple cases in a fast-paced healthcare environment
Comfortable working with confidential patient information
Reliable, detail-oriented, and adaptable to changing procedures

Preferred / Nice-to-Have Experience

Experience completing Medicaid applications through Maryland Health Connection (MHC)
Familiarity with Telligen's Qualitrac, for non-citizen Medicaid / Emergency Medical Services (EMS) eligibility review
Prior experience working with hospital referrals, Medicaid enrollment portals, or state eligibility systems
