Care Coordinator / Prevention Specialist
Outpatient Mental Health Clinic & School-Based Services
Hope Health Systems, Inc. is seeking a qualified and motivated Care Coordinator / Prevention Specialist to support children, adolescents, and families receiving outpatient and school-based behavioral health services. This role combines care coordination, prevention programming, and school-based collaboration across Baltimore City and Baltimore County Schools.
Key Responsibilities
- Develop and coordinate behavioral health prevention programs within Baltimore City and Baltimore County Schools
- Provide one-on-one care coordination, prevention services, and small group interventions to students and families at assigned school sites
- Serve as a liaison between families, schools, therapists, psychiatrists/NP providers, and community resources
- Review and coordinate internal referrals with school-based therapists to address identified client needs (e.g., identification documents, housing resources, benefits)
- Work collaboratively with treating psychiatrists/NPs and interdisciplinary teams to support treatment goals and objectives
- Conduct home visits when clinically or programmatically appropriate
- Maintain consistent communication with treating therapists through meetings, phone calls, or consultations, with required documentation of collateral contacts
- Track and report required prevention and service coordination data for Hope Health Systems, Behavioral Health Systems Baltimore, and county reporting requirements
- Participate in all staff meetings, evidence-based practice trainings, and CEU opportunities
- Maintain accurate, timely, and compliant documentation in accordance with OMHC and program standards
School-Based & Prevention Activities May Include
- Coordinating behavioral health services within school environments
- Supporting prevention initiatives focused on emotional wellness, coping skills, and early intervention
- Facilitating small groups and prevention-focused activities
- Supporting family engagement and advocacy within educational systems
- Monitoring follow-up on referrals, service engagement, and continuity of care
Job Requirements
- Education: Master’s degree in Social Work, Psychology, Counseling, Music Therapy, Art Therapy, Family Systems, or another behavioral health discipline
- Experience: Minimum of one year of experience in the mental health field providing individual, group, or family therapy (internships accepted)
- Ability to work effectively with children, adolescents, families, and school personnel
- Strong communication, organizational, and documentation skills
- Valid driver’s license, personal vehicle, and active automobile insurance
- Valid CPR and First Aid certifications
- Ability to pass state and federal criminal background checks
Preferred Qualifications
- Experience in outpatient mental health clinics (OMHC)
- Experience providing school-based behavioral health or prevention services
- Familiarity with Baltimore City and Baltimore County school systems
- Knowledge of local behavioral health and community resources
- Bilingual (Spanish preferred)

Job Types: Full-time, Part-time
Pay: $50,000.00 - $62,000.00 per year
